Make "Attach Fee Types" windows MUCH wider.

When you edit loan products, the windows for adding and removing fees should 1.5
or 2x wider than it is currently.

A basic name like "Loan Application Fee" barely fits in the window. If an MFI
creates "Loan Application Fee (1%)" and "Loan Application Fee (2%)", then the
key part of the name, the percentage, is hidden and you have no idea which one
you're adding. (Until you go to the Preview screen.)

(And to make matters worse, you can't then edit the name in the fee definition
so you have to cancel the fee and start a new one, very user unfriendly and
tedious).
